Kožní projevy lupus erythematodes

SOUHRN

Lupus erythematodes (LE) je autoimunitní onemocnění s různými klinickými projevy od izolovaného kožního onemocnění, tj. kožního LE (cutaneous lupus erythematosus, CLE), až po systémovou chorobu postihující více orgánů, tj. systémový LE (systemic lupus erythematosus, SLE). Kožní léze asociované s LE lze rozdělit na LE-specifické a LE-nespecifické. Léze specifické pro LE zahrnují akutní kožní LE (acute cutaneous lupus erythematosus, ACLE), subakutní kožní LE (subacute cutaneous lupus erythematosus, SCLE) a chronický kožní LE (CCLE). Mezi LE-nespecifické kožní léze se řadí ty kožní projevy, které sice často nalézáme u pacientů se SLE, ale lze je pozorovat i u jiných onemocnění, například u alopecie, některých cévních změnách nebo při kalcinóze kůže. K prevenci řady kožních projevů LE je nutná důsledná fotoprotekce, léčebně pak používáme podle rozsahu a závažnosti topické nebo systémové glukokortikoidy, antimalarika a další imunomodulační a imunosupresivní léky.

SUMMARY

Lupus erythematosus (LE) is an autoimmune disease with a variety of clinical manifestations, from isolated skin disease, ie cutaneous lupus erythematosus (CLE), to systemic disease affecting multiple organs, ie systemic lupus erythematosus, SLE). LE-associated skin lesions can be divided into LE-specific and LE-non-specific. LE-specific lesions include acute cutaneous lupus erythematosus (ACLE), subacute cutaneous LE (subacute cutaneous erythematosus, SCLE) and chronic cutaneous LE (CCLE). LE-nonspecific includes those skin manifestations that are often found in patients with SLE, but can also be observed in other diseases, such as alopecia, some vascular changes or skin calcinosis. Thorough photoprotection is necessary to prevent a number of skin manifestations of LE, and topical or systemic glucocorticoids, antimalarials and other immunomodulatory and immunosuppressive drugs are used therapeutically, depending on the extent and severity.
