Léčba bolesti u pacientů s hemofilií

SOUHRN

Hemofilie je dědičné onemocnění, které postihuje z více než 90 % chlapce. Hlavním příznakem je krvácení, nejčastěji do kloubů. Přestože se péče o tyto pacienty stále zlepšuje, krvácení do kloubů se nedá zcela zabránit. Zvláště u těžkých forem je časté a prudké. Změny v kloubech způsobené krvácením vedou k chronické bolesti, deformitám a těžkým artropatiím, které značně zhoršují kvalitu života těchto nemocných. Preventivní a aktuální náhrada chybějícího srážecího faktoru zmírňuje akutní bolest i následky krvácení. Přispívá však také k tvorbě inhibitoru, který zhoršuje stav a průběh onemocnění. V centrech pro hemofilii je léčba zaměřena hlavně na profylaxi a substituci faktoru, systematická léčba bolesti včetně farmakoterapie je však u 30-50 % pacientů nedostatečná.

SUMMARY

Haemophilia is a hereditary disease that affects mostly boys (in more than 90 % cases). The main symptom is bleeding, most commonly into the joints. Although care for these patients is still improving, bleeding in the joints cannot be completely prevented. Especially in the heavy forms, bleeding is frequent and severe. Changes in the joints caused by bleeding lead to chronic pain, deformities and severe arthropathy, which greatly impair the quality of life of haemophiliac patients. Preventive and current replacement of the missing clotting factor alleviates both acute pain and bleeding. However, it also contributes to the formation of an inhibitor that worsens the condition and course of the disease. In haemophilia centers, treatment is mainly focused on clotting factor prophylaxis and substitution. However, systematic pain management, including pharmacotherapy, is inadequate in 30-50 % of patients.
