Léčba srdečního selhání u křehkého nemocného

SOUHRN

Podáváme přehled přístupu k léčbě „křehkého“ nemocného se srdečním selháním. Základem léčby podle nových doporučeních pro diagnostiku a léčbu srdečního selhání z roku 2021 jsou inhibitory angiotenzin konvertujícího enzymu / blokátory receptoru AT, pro angiotenzin II (sartany) s možností nahrazení blokátory receptoru AT, pro angiotenzin II a neprilysinu (sacubitril-valsartan), dále beta-blokátory, glifloziny a blokátory mineralokortikoidních receptorů, při známkách srdečního městnání také diuretika. Dávky léků je však nutné přizpůsobit hemodynamickému stavu nemocného, je třeba zvážit kontrolu, aby léky pravidelně užíval, včetně kontroly nežádoucích účinků. Také bereme v úvahu řadu komorbidit, kterými křehký nemocný trpí. Velmi pečlivě budeme zvažovat indikace implantabilního defibrilátoru.

SUMMARY

We provide an overview of the approach to the treatment of a “fragile” patient with heart failure. The basis of treatment according to the new recommendations for the diagnosis and treatment of heart failure from 2021 are angiotensin-converting enzym inhibitors / sartan with the possibility of replacement by angiotensin II receptor AT, antagonists and neprilysin (sacubitril-valsartan), beta-blockers, gliflozins and mineralocorticoid receptor antagonists, and diuretics. However, it is necessary to adjust the dose of drugs to the hemodynamic condition of the patient, to take into account the control so that he receives the drugs regularly, including the control of side effects. We also take into account a number of comorbidities that a frail patient suffers from. We will consider very carefully the indications for implantable defibrillator.
