SOUHRN

Specifická alergenová imunoterapie (SAIT) je jediný kauzální způsob léčby alergie, který zasahuje přímo do imunopatologických mechanismů, jejichž důsledkem jsou klinické symptomy alergie pacienta. V současnosti je SAIT indikována k léčbě alergické rinokonjunktivitidy, alergického astmatu a alergie na jed blanokřídlého hmyzu projevující se anafylaktickou reakcí. Během léčby se do organismu alergika v pravidelných časových intervalech vpravují definované dávky terapeutického alergenu, na který je tento pacient přecitlivělý. Cílem je snížit reaktivitu jedince na konkrétní alergen. Formy aplikace v současnosti jsou injekční subkutánní forma a forma sublingvální buď kapková, nebo tabletová. Doba léčby je 3-5 let. Budoucnost v oblasti alergenové imunoterapie bude patřit přípravkům s vyšší imunogenicitou a nižší alergenicitou, preferenčně v neinjekční tabletové formě.

SUMMARY

Specific allergen immunotherapy (SAIT) is the unique treatment affecting directly the immunopathological mechanisms of IgE mediated allergies. Allergic rhinitis, allergic asthma bronchiale and hymenoptera venom allergy with the history of anafylaxis are the indications for SAIT nowadays. The treatment consists of the consecutive application of causal allergen standardized extract in the ascending order. The aim is to lower the hypersensitivity to the causal allergen. The application routes of SAIT are sublingual (SLIT) or subcutaneous (SCIT). The duration of treatment varies from 3 to 5 years. The future perspectives in SAIT intend to alleviate the allergenicity and to enhance the immunogenicity of SAIT, preferentially via the SLIT application route.
