Stillova choroba dospělých komplikovaná rozvojem akutního jaterního selhání s nutnou transplantací jater

SOUHRN

Předkládáme případ 46leté ženy s bolestmi v krku, kožním rashem, artritidou, horečkou, hepatosplenomegalií, leukocytózou, poklesem hmotnosti a hyperferritinemií. Per exclusionem byla stanovena diagnóza Stillovy choroby dospělých. Přes úvodní účinnost léčby kortikosteroidy došlo následně k rozvoji akutního selhání jater s nutností urgentní transplantace jako život zachraňujícího výkonu. Pacientka je již dva roky po transplantační léčbě v dobrém klinickém stavu, s normální funkcí jater a bez jiných klinických projevů Stillovy choroby.

SUMMARY

We present the case of 46-year old woman with sore throat, high fever, transient cutaneous rash, arthritis, hepatosplenomegaly, weight loss and hyperferritinaemia. Per exclusionem was diagnosed Still's disease of adult-onset. Despite the initial positive response to corticosteroid the acute liver failure occurred, this resulted in urgent liver transplantation as lifesaving treatment. The patient is currently 2 years after the abovementioned transplantation in a good shape, with normal liver function and without any other clinical signs of Still's disease.
